Cancellation & No Show Policy
Each ‘no show’ or ‘same day cancellation’ of a scheduled appointment will result in a fee of $40 per occurrence that the patient, not their health insurance plan, will be required to pay at the time of their next clinic visit. A ‘no show’ means a patient who fails to arrive for a scheduled appointment and a ‘same day cancellation’ means a patient who cancels an appointment less than 24-hours before their scheduled visit.
Insurance, Payment & Assistance
We accept all major insurances, including Med-QUEST. Payment is expected at the time of service. Cash, check, debit, and major credit cards are accepted types of payment. Payment plans may be arranged for those facing economic hardship. Depending on your family income and family size, you may qualify for reduced charges under our sliding fee discount program.
