- Mon-Thur: 7AM to 6PM
- Closed (lunch): 1PM to 2PM
Medical Records
- You can view your medical records via the Patient Portal. Our practice code is DEEFCD.
- New patients, to request a copy of your medical records be sent to us, fill-out the Medical Release Form form and submit the completed form to your provider.
- Current patients, to release your medical record to another provider, complete the Patient Authorization to Release Medical Information form and drop it off at the clinic or send via secured email to patientservices@maikaihealth.org.
Billing Questions
- To pay your bill online, please use the Healow Pay Portal. You will need your personal statement code located in the ‘statement detail’ section on your bill.
- If you have questions about your bill, please contact AkamaiMD, our contracted medical billing service, at (808) 238-0947, Monday – Friday, 9 AM to 4 PM.
- For payment plan options, please email us at finance@maikaihealth.org or ask our front desk staff for assistance.
