Chronic Care Management (CCM)<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

\n\t\t\t\t\t\t\t\t\t

Our Pilina Care CCM program is an innovative partnership between our clinic and KTA Pharmacy, a\u00a0community-based pharmacy. KTA Pharmacy works closely with our providers and pharmacy staff\u00a0to ensure seamless care coordination of Pilina Care CCM patients. The program provides end-to-end treatment\u00a0strategies in a collaborative care model, which includes primary medical and\u00a0mental health care, integrated pharmacy, and remote monitoring.<\/p>

The Pilina Care program allows each organization to share resources and expertise, thereby:<\/p>

  • Expanding access and faster time to critical intervention<\/span> as Pilina Care CCM patients will\u00a0have access to health care and support services in-clinic, outside of an office visit.<\/li>
  • Improving patient engagement<\/span> due to the high level of patient\/care team interaction from\u00a0month to month.<\/li><\/ul>
    Remote Patient Monitoring (RPM)<\/strong><\/h5>

    In partnership with Medicare and managed in association with Accuhealth, our RPM program’s goal is to assist our patients with managing their chronic illnesses.<\/p>

    Managing chronic conditions, such as diabetes, hypertension, heart failure, COPD and others, requires regular monitoring of your blood pressure, blood sugar, weight, and\/or temperature. Our RPM program will make this much more convenient by allowing our RPM enrolled patients to do this in the comfort of their home.<\/span><\/span><\/p>

    Participating patients will receive their equipment by mail and Accuhealth will contact the patient to train them on how to use the equipment. The medical device will be connected directly to the patients Electronic Health Record. The program is completely optional and the\u00a0<\/strong>best news is that all of this is covered by insurance! <\/strong>This includes the device, the set- up and the monitoring. Of course, the rules of insurance will still apply, such as deductibles, co- insurance, authorizations, etc.<\/span><\/p><\/div>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t

    Outreach Clinic<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

    Our Pilina Care Outreach Clinic offers the following services in various locations and events throughout East Hawaii.<\/p>\n

      \n
    • Health and wellness information and education<\/li>\n
    • Blood pressure screening<\/li>\n
    • COVID-19, Monkeypox, and flu vaccinations<\/li>\n
    • Among other health screenings and tools<\/li>\n<\/ul>\n

      We also plan to grow our Outreach Clinic in 2023 to better meet the health care needs of our more rural communities. Most Outreach Clinics will bundle health education with certain medical services, workshops, and trainings. In addition to health education, we will offer vaccinations (Hepatitis A, A\/B, COVID-19, etc.) wound care, and screenings for blood pressure, hepatitis, diabetes, Influenza, among others. We will also host trainings, including Mental Health First Aid tools, wound self-care and when to seek help, among others.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t
